Required Before Arrival
Waterfowl hunters accessing Five Dot property through an outside guide or group must complete and submit this Access License before arriving. This applies to every visit. The license is valid through the end of the calendar year it’s signed in, and must be renewed annually. Big-game hunters do not use this form – releases for big-game hunts are coordinated directly as part of the trip-planning process.
